Application

Basic Qualifications and Eligibility Criteria:

1. Citizenship and Residency

• U.S. Citizenship or Legal Permanent Residency: Applicants must be either citizens or legal, permanent residents of the United States.
• State Residency: Applicants must reside in either South Carolina or North Carolina. Proof of residency, such as a driver’s license, utility bill, or lease agreement, may be required.
• Faith and Blessings Foundation welcomes diversity and encourages all eligible applicants to apply regardless of race, religion, or gender.

2. Age Requirement

• Applicants must be between 21 and 45 years old, to ensure they are at an optimal age for fertility treatments.

3. Financial Need

• Applicants must demonstrate financial need, which could involve providing details about income, insurance coverage, and previous expenditures on fertility treatments. This may include submission of tax returns, pay stubs, or a detailed financial statement.

4. Insurance Coverage

• Applicants must demonstrate either a lack of insurance coverage for fertility treatments or insufficient coverage, where the costs of required treatments exceed what insurance will cover.

5. Treatment Plan

Fertility Treatment Outline: Applicants are required to provide a detailed treatment plan prepared by a licensed fertility specialist. This plan should clearly outline the specific treatments recommended, along with a breakdown of the associated costs.
Surrogacy or Adoption Agency Details: Applicants pursuing surrogacy or adoption must also provide information about the agency they are using, including the agency’s name, contact details, and a summary of the services they will provide.

6. Consent to Follow-Up

• Agreeing to provide follow-up data on the outcomes of the treatments funded by the grant. This helps the granting organization assess the impact of their program and make necessary adjustments for future grants.

7. Ethical and Legal Compliance

• Applicants must agree to adhere to ethical and legal standards in all procedures involving fertility treatments.

Application Process

Form Submission: Complete and submit the official grant application form provided by the Faith and Blessings Foundation. There is a $75 fee to cover administrative costs.
Documentation: Provide all required documentation that supports the qualifications listed, such as proof of residency, medical diagnosis, financial information, and treatment plan.
Interview: If additional information is required, we may request an interview with the applicants to better understand their situation and needs.

Special Considerations

• Faith and Blessings Foundation may prioritize applicants based on additional factors such as military service, public service, the number of prior attempts at fertility treatments, or specific health conditions that complicate fertility.

Personal Statement

Help us get to know you and your situation. Personal statement should include the following (3 pages max, photos are allowed):

Background: Start by briefly introducing yourself and your partner (if applicable). Include relevant background information that may impact your fertility journey, such as age, occupation, and where you live.

Fertility Challenges: Describe the specific fertility challenges you have faced. Be open about the emotional and physical toll these challenges have taken on you and your family.

Medical Interventions: Detail any previous fertility treatments you have undergone, such as medications, surgeries, IUI, IVF, etc. Mention the outcomes and how these have impacted your decision to seek further help.

Financial Burden: Explain the financial impact these treatments have had on your life, including any sacrifices made to afford them.

Emotional and Social Impact: (Optional) Share the emotional and social repercussions of struggling with infertility. Discuss any feelings of isolation, depression, stress, or pressure from family or society.

Personal Growth: (Optional) share how facing these challenges has led to personal or relational growth.

Financial Need: Clearly articulate why you need financial assistance. Provide specifics about your current financial situation that make paying for further treatment difficult or impossible.

Lack of Insurance Coverage: If relevant, explain how your insurance (or lack thereof) affects your ability to pursue treatment.

Future Plans: Describe what receiving this grant would mean for you and your family’s future. Explain your hopes and plans for building your family if you were to receive the grant.

Impact of the Grant: Discuss the potential impact of the grant on your life and the lives of your future children.
